TriGenerational Homecare

We hope that every elderly in Singapore is able to experience the protection, care and love of a family! As healthcare students, we aim to serve the medical, psychological and social needs of elderly in the Northwest through long-term holistic care. We also hope to educate, enable, and empower young adults to become champions of their communities. Our project encompasses 3 different generations - the elderly, university students in healthcare faculties, and secondary school students. Last year, TriGen is proud to celebrate our 10th anniversary, where we celebrate a decade of intergenerational care, connection, and community impact.

During the course of the cycle, each team will conduct regular home visits to follow up on their elderly patients. Throughout this period, the patient's progress will be monitored and documented. There will be a mid-cycle Multi-Disciplinary meeting where Team Leaders will have the opportunity to discuss their patient's progress with healthcare professionals and work with the professionals to improve their care plan.
